How can I get Hostmon to report from date X to date y ?

HostMonitor creates reports that show CURRENT status of the tests.
If you want to analyze some time interval, use Log Analyzer

Also, am I able to use the %Threshold% macro for inserting into SQL logs ?

There are no such variable.
Use %AlertThreshold%
