I have seen that as remote hosts are assigned a new dynamic address, HM is still trying to access them via a test at a cached address that is no correct. This causes a the test (check for process) to show unknown status.

This is because HM is now checking for the process on the wrong host.

Has anyone else had to deal with this.

My config:
XPSP2 FW disabled
Static IP no WINS specified
enabled NetBIOS over TCP/IP
running HM as a service version 5.12

nslookup is accurate, cannot seem to effectively clear the netbios cache without a reboot.

HostMonitor does not use internal cache for IP addresses. You are right, it looks like netbios cache problem.
Try to use "NbtStat -R" or "NbtStat -RR" command

If dns is correct (and sufficient), you could disable netbios.
