Other than clicking Monitoring > Disable Alerts is there any other way that alerts can become disabled?? eg. restart of HM?? acknowledging bad status with "acknowledge and stop alerts"???

To acknowledge the failed test with "acknowledge and stop alert" option you may use HMS script with "AckTestStatus" command with "StopAlerts" parameter. http://www.ks-soft.net/hostmon.eng/mframe.htm#actions.htm#actScript

You may execute HMS script using menu "File > Execute script" or using "Execute HMS script" action.

My apologies, maybe I didn't explain properly. We found recently that when tests failed, the associated alert action (sound, message) didn't execute. After digging around I noticed that (and noticing a big button on the toolbar to switch them back on ) alerts were disabled in the monitoring menu.
Could this have happened by any means other than someone clicking Monitoring > Disable Alerts ?

No, it could not. There are no any automatical possibility to disable alerting. Someone should manually do it using menu "Monitoring > Disable alerts" in HostMonitor and RCC, by pressing "Stop Alerts" button in Web page, produced by Web Service, using "disable alerts" command with Telnet Service or by enabling "Disable alert reaction" option on HostMonitor's menu "Options > Behavior" tab. Also you may disable alert for some tests using "Acknowledge and stop alert" option. "Acknowledge and stop alert" option also could be invoked automatically for certain tests using HMS Script.
