No, we do not see any problems. May be HostMonitor cannot record info into system log either? Could you stop and start monitoring and check system log (C:\Program Files\HostMonitor5\Logs\syslog.htm)? You should see records like "Monitoring stopped", "Monitoring started"
When I modified the type of reply variable the sql query test works fine, when I click on test button in advanced log option HM reply Query executed and I have one log in table.
But when I save settings options all tests don't records in table.

Yes I can see records Monitor stopped Monitor started when I restart HM.
Could tell me the steps to configure HM to work with database, maybe I forgot somethings?

Looks like you have done it correctly. If Test button works fine, it means ODBC logging was setuped correctly. Very strange.
Such behaviour might be only in one case: in test properties window you unmarked "Use common log" check box. To write test info into log file this check box must be marked. Coud you check it?
